摘要：Chinese medical system has featured overloading tier-3 hospitals and underutilized primary facilities. After 2009, several measures have been taken to induce patients to primary outpatients. Our research design exploits a sharp increase in coinsurance for patients aged below 65 when paying tier-3 first-contact visits. Utilizing this age discontinuity, we find that increased copayment when bypassing primary facilities increase primary first contacts by 4 percentage point. In addition to that, patients' medical expenses per visit decrease.  Second, most patients with chronic conditions (e.g. diabetes, hypertension) adjust their first contact choices accordingly, while those with acute and severe diseases are not responsive. Third, after turning to primary hospitals, patients tend to seek preventive care.
