摘要(演讲一): While prior research has examined the impact of financial and non-pecuniary incentives on physician behavior, there is a paucity of literature exploring the relationship between working conditions and physician performance. This study addresses this gap by investigating the effect of a hospital open hours reform on physician productivity and quality of care. Specifically, we analyze high-frequency data on daily outpatient records, including doctor diagnoses, prescribed medications, and medical orders, at a representative hospital in southwestern China. Our results show that advancing clinician afternoon hours by one and a half hours while maintaining morning hours led to a nearly 20% decrease in physician productivity, though this effect declined within six weeks of implementation. We also evaluate physician quality by examining total outpatient spending, prescriptions, and medical orders. Our findings suggest that there were marginal decreases in these measures following the hospital open hours reform. These findings underscore the significance of understanding how working conditions can shape physician performance and the quality of care delivered to patients.
摘要(演讲二):This research examines the relationship between temperature and hospitalization from Emergency Department(ED). Using records of inpatient visits transferred from ED in 194 hospitals from 89 cities in 25 provinces across China from year 2013 to 2022, we find hospitalization from ED decreases by 13.46% on a day with mean temperature under -6°C and increases by 8.51% on a day with mean temperature over 30°C, compared to a benchmark of 13°C. After accounting for the temperature in a previous 30-day period, we find a cumulative effect of decrease by approximately 9.6% and increase by 2.9% in the coldest and hottest days respectively. We also find that ED-inpatient visits of the elderly is more sensitive to temperature change, and the decrease in healthcare utilization is of smaller magnitude in areas with central heating.
