摘要:Cognitive impairment has large impact on individuals’ decision making, independent living, and wellbeing. Identifying early signals of dementia risk may offer people more time to prepare for the future, helping to delay the onset or slow the progression of dementia. Using the 1995-2018 waves of Health and Retirement Study and linked Medicare claims data, we offer novel evidence on the impacts of dementia and its cognitive impairment precursors on a rich set of preventive care utilization and health behaviors. Leveraging both within- and between-individual variations in an event study design, we characterize long-term dynamic changes in preventive care and health behaviors relative to the incidence of dementia and find early behavioral indicators of the disorder. We show that relative to the group of never demented, people with dementia have consistent and escalating declines in the use of cholesterol test, mammogram, prostate test, and dental visit around the incidence of dementia. Significant declines are also found in physical activities and social engagement. Importantly, we demonstrate that the behavioral changes can occur up to 6 years before the incidence of dementia; and these patterns are absent in other chronic or acute conditions. The results are robust to sample selection, model specification, and more flexible controls of aging and cohort effects.

主讲人简介:Xi Chen, PhD, is an associate professor of health policy and economics at Yale University. His research integrates causal inference, machine learning, and epidemiological approaches to evaluate policies on population aging, life course health, and global health systems. He is an Editor at the Journal of Population Economics, a consultant at the United Nations, the World Bank, Research Fellow at IZA Institute of Labor Economics, former President of the China Health Policy and Management Society, U.S. National Institutes of Health’s Butler-Williams Scholar and Claude D. Pepper Scholar, research affiliate of Cornell Institute on Health Economics, adjunct professor at Peking University, fellow Cluster Lead at the Global Labor Organization, and a member of the editorial board at China CDC Weekly. His research funded by the NIH/NSF/USDA has produced 106 original studies that appear in major science, economics, and medical journals, such as Lancet, PNAS, JAMA, JEEM, JAERE, JoPE, PLoS Medicine, SSM, AJAE and JEoA, recognized through numerous awards such as the Kuznets Prize and George F. Warren Award, and widely covered in global media. He is a commentator at BBC, Bloomberg, CNN, and writes opinion pieces for NYT. He obtained a PhD in Applied Economics from Cornell University.
